What is a Community Health Worker (CHW)?

CHWs are trusted members of the community who shares ethnic, language, and cultural background and norms with the community they serve.  CHWs 

  • Mediate between the healthcare, social services, and the community members 

  • Close the gap in access to needed services and resources by the community they serve

  • Build capacity through health education and self-sufficiency for better socioeconomic and health outcomes

  • Become the community's advocate and liaison through outreach, as patient navigators, health information and education
